Mae Ashford (71) from Blackpool in Great Britain was diagnosed with a brain tumor in 2010, after suffering from headaches and seizures.
A series of MRI scans showed that the tumor was growing. However, the 71-year-old was not offered the surgery until five years later.

Ashford later died of a stroke.
According to the country’s health authorities, who investigated the death, Ashford died as a result of not being operated on in time.
The investigation found that health care personnel did not follow up on MRI scans well.
Experts say Ashford should have had the surgery at least three years ago, before the tumor had grown large enough to affect several areas of the brain.
– He drove for four years
It was Mae Ashford’s husband, Alan Ashford, who complained about the matter to the Ombudsman.
He says the family was left without answers.
– My wife suffered terribly from the consequences of the tumor for more than four years. It was clear to us in the family when we read the MRI reports that the tumor was very serious, he told Sky News.

The tumor must be removed before it touches the neck artery. Not to do so is a great mystery to us.

Ombudsman Rob Behrens says the case is an important reminder to improve working with MR images.
He told Sky News: “Unfortunately, our case management shows that Ashford is not the only person who lost his life due to scan and x-ray errors.”
A spokesperson for Lancashire Teaching Hospitals acknowledges the findings of the NHS report and admits they failed to treat May Ashford.
